OCR description
The covered entity (CE), Native Village of Eyak Ilanka Community Health Center, reported that its business associate experienced a data breach incident that affected the protected health information (PHI) of 512 individuals. The PHI involved included names, dates of birth, diagnoses/conditions, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE and BA implemented additional administrative, technical, and security safeguards to better protect PHI.
