OCR description
The covered entity (CE), The Physical Medicine & Rehabilitation Center, reported that it experienced a cybersecurity incident that affected the protected health information (PHI) of 4,083 individuals. The PHI involved included names, addresses, dates of birth, Social Security and drivers' license numbers, financial information, diagnoses, and other treatment information. The CE notified HHS, the media and affected individuals. In response to the breach, the CE implemented additional technical and security safeguards.
