OCR description
The covered entity (CE), BrightStar Care, reported that an employee was the target of an email phishing scheme that compromised the protected health information (PHI) of approximately 1,187 individuals. The PHI involved names, addresses, dates of birth, drivers’ license and social security numbers, claims and financial information, and diagnoses and conditions. The CE notified HHS and the affected individuals. In its mitigation efforts, the CE implemented several administrative and technical safeguards.
