OCR description
The covered entity (CE), Family Medical Center, reported that it was the subject of a ransomware attack that affected the protected health information (PHI) of 2,100 individuals. The PHI involved included names, Social Security numbers, addresses, drivers’ license numbers, dates of birth, claims information, diagnoses/conditions, lab results, and medications. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards, and retrained workforce members to better protect its PHI.
