OCR description
Potomac Medical Associates, the covered entity (CE) reported that an employee sent an email containing the protected health information (PHI) of 2,876 individuals to the wrong recipients. The PHI involved included names and email addresses. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, a call center was established for questions and concerns and the CE implemented additional administrative safeguards to better protect PHI. Staff were retrained on the requirement to protect and secure sensitive data.
