OCR description
The covered entity (CE), Kaiser Foundation Hospitals, reported that several employees were the targets of an email phishing scheme that compromised the protected health information (PHI) of 44,600 individuals. The PHI involved included demographic and clinical information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE changed password/strengthened password requirements, implemented new technical safeguards, and revised policies and procedures.
