OCR description
Embody Performance & Recovery, the covered entity (CE), reported that an employee emailed the protected health information (PHI) of 1,215 individuals without using the blind carbon copy function. The PHI involved included email addresses. The CE notified HHS, affected individuals, and the media of the breach. In response to the breach, the CE implemented additional administrative safeguards and performed a new risk analysis. OCR provided technical assistance to the CE.
