OCR description
The covered entity (CE), Planned Parenthood of Montana, reported that it experienced a hacking attack that compromised the protected health information (PHI) of 56,917 individuals. The PHI involved included names, addresses, dates of birth, health insurance information, diagnoses, medications, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice of the breach on its website. In response to the breach, the CE implemented additional administrative and technical safeguards to better protect its PHI.
