OCR description
The covered entity (CE), Orthopedics Rhode Island, reported that it was the subject of a ransomware attack that affected the protected health information (PHI) of 377,731 individuals. The PHI involved included names, addresses, dates of birth, social security numbers, diagnoses/conditions, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE provided complimentary credit monitoring to the affected individuals and implemented additional physical and technical safeguards.
