OCR description
Hopscotch Health Management, the business associate (BA), reported that it experienced a cyberattack that compromised the protected health information (PHI) of 4,945 individuals. The PHI involved included names, addresses, dates of birth, diagnoses, conditions, lab results, medications, and other treatment information. The BA notified HHS, affected individuals, and the media. The BA took steps to improve its physical security.
