Physicians' Primary Care of Southwest Florida

The U.S. Department of Homeland Security alerted Physicians’ Primary Care of Southwest Florida, the covered entity (CE) to suspicious traffic by a cyber threat actor group. The CE investigated and determined that the threat actor group had hacked its computer network, and the breach affected the protected health information of 170,653 individuals. The PHI involved demographic and treatment information. The CE provided breach notification to affected individuals, the media, HHS, and posted substitute notice on its website. In response to the breach, the CE implemented a series of improved technical safeguards to further protect PHI.
