OCR description
The covered entity (CE), Western Montana Mental Health Center, reported that it was the subject of a ransomware attack that affected the protected health information (PHI) of 86,758 individuals. The CE notified HHS and the affected individuals. In response to the breach, the CE implemented additional technical and security safeguards.
