OCR description
The covered entity (CE), VPS of MI PLLC, reported that it was the target of a cyber-attack that compromised the protected health information (PHI) of 20,604 individuals. The PHI involved clinical, demographic, and financial information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional technical safeguards. OCR provided technical assistance to the CE.
