Equinox, Inc.

The covered entity (CE), Equinox, reported that it experienced a ransomware incident that affected the protected health information (PHI) of 21,565 individuals. The PHI involved included names, addresses, dates of birth, Social Security and drivers’ license or other government identification numbers, passport numbers, financial information, health insurance information, diagnoses, medications, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional administrative, technical, and security safeguards. Staff were also provided training on its requirements to protect and secure PHI.
