OCR description
The business associate (BA), HealthFund Solutions, LLC. reported that it experienced a hacking incident that affected the protected health information (PHI) of 5,198 individuals. The PHI involved included clinical, demographic, and financial information. The BA notified HHS, affected covered entities, affected individuals, and the media. In response to the breach, the BA offered free credit monitoring to affected individuals, and implemented administrative, technical and security safeguards.
