OCR description
The covered entity (CE), Enso Counseling Group, reported that an employee did not use the blind carbon copy function and made the protected health information (PHI) of 943 individuals viewable by others. The PHI involved included names and email addresses. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE implemented additional administrative and security safeguards to protect and secure sensitive data.
