OCR description
The covered entity (CE), Pavilion of Bridgeview, reported that its business associate (BA) experienced a breach incident that compromised the protected health information (PHI) of 1,407 individuals. The PHI involved included names, addresses, birthdates, social security numbers, diagnoses, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the BA provided free credit monitoring and implemented additional administrative and technical safeguards.
