OCR description
The covered entity (CE), Hardin County Emergency Medical Services, reported that an employee was the target of an email phishing scheme that affected the protected health information (PHI) of 1,046 individuals. The PHI involved demographic, clinical, and financial information. The CE notified HHS, affected individuals, and the media. In response to the breach the CE implemented additional technical safeguards and retrained the involved employee. OCR provided technical assistance regarding the HIPAA Security Rule and the Breach Notification Rule.
