OCR description
Veterans Health Administration, the covered entity (CE), reported that its business associate (BA) experienced a ransomware attack that compromised the protected health information (PHI) of 2,302 individuals. The PHI involved included names, medical record information, Social Security numbers and treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the BA implemented additional administrative, technical, and security safeguards.
