OCR description
The covered entity (CE), Center for Vein Restoration, reported that it experienced a cyberattack that compromised the protected health information (PHI) of 446,094 individuals. The PHI involved included names, home addresses, dates of birth, Social Security and drivers’ license numbers, medical record numbers, diagnoses, lab results, medications, health insurance information, and financial information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach the CE implemented additional administrative, technical, and security safeguards.
