The business associate (BA), Ott Cone & Redpath, reported that employees were the subjects of an email phishing attack that affected the protected health information (PHI) of 22,171 individuals. The PHI involved included names, addresses, dates of birth, social security numbers, claims and financial information, diagnoses, conditions, and other treatment information. The BA notified HHS, affected individuals, the media, and posted substitute notice. In response to the breach, the BA adopted encryption technologies, strengthened its security policy and procedures, and implemented additional technical safeguards.
