OCR description
The covered entity (CE), CHCM, Inc. dba College Hospital Costa Mesa, reported that it experienced a hacking incident that affected the protected health information (PHI) of 38,695 individuals. The PHI involved included demographic and clinical information. The CE notified HHS, affected individuals, the media, and provided individuals with free credit monitoring. In response to the breach, the CE implemented new technical safeguards.
