OCR description
The covered entity (CE), Golden Age Home Health, reported that it was the subject of a ransomware attack that affected the protected health information (PHI) of 3,223 individuals. The PHI involved included names, Social Security numbers, addresses, drivers’ license numbers, and dates of birth. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE provided complimentary credit monitoring services to the affected individuals and implemented additional administrative, technical, and security safeguards, and retrained workforce members to better protect its PHI.
