OCR description
Cardiology Associates of Mobile, the covered entity (CE), learned that it experienced a cybersecurity incident that affected the protected health information (PHI) of 172,678 individuals. The PHI involved included names, dates of birth, addresses, telephone numbers, Social Security numbers, claims information, lab results, medications, diagnoses and conditions, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice. In response to the breach, the CE implemented additional administrative, technical, and security safeguards.
