OCR description
The covered entity (CE), BCBS of TX, reported that an employee of its business assocate (BA) mailed protected health information (PHI) of 1,028 individuals to the incorrect addresses. The PHI involved included clinical, demographic, and financial information. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards, and retrained workforce members to better protect its PHI.
