OCR description
The covered entity (CE), Menorah Life, reported that its business associate (BA) experienced a cybersecurity incident that compromised the protected health information (PHI) of 2,735 individuals. The PHI involved included names, Social Security numbers, addresses, birthdates, medications, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the BA implemented additional administrative and technical safeguards.
