OCR description
The covered entity (CE), Kansas City Hospice, reported that it experienced a hacking attack that compromised the protected health information (PHI) of 3,621 individuals. The PHI involved names, Social Security numbers, and diagnoses and conditions. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards, and retrained workforce members to better protect PHI.
