OCR description
The covered entity (CE), Dignity Health Lassen Medical Clinic, reported that it experienced a ransomware incident that affected the protected health information (PHI) of 64,946 individuals. The PHI involved included names, addresses, dates of birth, social security numbers, drivers’ license numbers, and financial and health insurance information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards and provided free credit monitoring to affected individuals.
