OCR description
Youngs Counseling, the covered entity (CE), reported that an employee disclosed the protected health information (PHI) of 790 individuals when emailing a newsletter and exposing all of the emails. The PHI involved email addresses only. The CE notified HHS, affected individuals, and the media. In response to the breach and OCR’s investigation, the CE revised its policies and procedures, implemented additional technical safeguards, sanctioned its workforce member and retrained staff.
