Tycon Medical Systems, Inc.

The covered entity (CE), Tycon Medical Systems, Inc., reported that an unauthorized party gained access to its network environment, accessing the protected health information (PHI) of 112,847 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, medical information, and health insurance information. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI, including conducting a risk analysis and asset inventory, strengthened its policies and procedures, and implemented multifactor authentication.
