OCR description
The business associate (BA), PrintComm, Inc., reported that an employee mailed the protected health information (PHI) of 3,864 individuals to the wrong recipients. The PHI involved included names and claims information. The BA notified HHS, the affected individuals, and the media. In response to the breach, the BA provided complimentary credit monitoring services to the affected individuals and implemented additional administrative, technical, and security safeguards to better protect PHI.
