OCR description
The covered entity (CE), TruBridge reported that it experienced a hacking incident that affected the protected health information (PHI) of 3,855 individuals. The PHI involved included names and patient account information. The CE notified HHS and affected individuals. In response to the breach, the CE implemented additional administrative, technical, and security safeguards.
