The covered entity (CE), Indiana University Health, Affiliated Covered Entity, reported that an employee was the subject of an email phishing scheme that compromised the protected health information (PHI) of approximately 1,446 individuals. The PHI involved names, dates of birth, addresses, social security and drivers’ license numbers, claims information, diagnoses/conditions, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE provided security awareness training to its workforce and enhanced its access controls and email security to better protect its PHI. OCR provided technical assistance to the CE regarding the HIPAA Rules.
