OCR description
The business associate (BA), McNall & Associates, P.C., reported that it experienced a hacking incident that affected the protected health information (PHI) of 10,175 individuals. The PHI involved included clinical and demographic information. The BA notified HHS, affected individuals, the media, and provided individuals with free credit monitoring. In response to the breach, the BA implemented new technical safeguards.
