BayMark Health Services, Inc.

The business associate (BA), BayMark Health Services, reported that an unauthorized individual gained access to its system through brute force and launched a ransomware attack affecting the electronic protected health information (ePHI) of approximately 265,701 individuals. The ePHI involved included names, dates of birth, social security numbers, drivers’ license numbers, insurance, and medical information. The BA notified HHS, the affected covered entities, the affected individuals, and the media. In response to the breach, the BA implemented additional system monitoring safeguards and additional technical safeguards.
