LifeBridge Health, Inc

LifeBridge Health, Inc., the covered entity (CE), reported that multiple employees were the targets of an email phishing scheme that affected the protected health information (PHI) of 25,895 individuals. The PHI involved included names and addresses, birthdates, Social Security and drivers’ license numbers, medications, claims and health insurance information, and other treatment information. The CE notified HHS, affected individuals, media outlets, and posted substitute notice on its website. In its mitigation efforts, the CE implemented provided complimentary credit monitoring services and implemented additional administrative, technical, and security safeguards. Staff were retrained on email security.
