OCR description
The covered entity (CE), Building Service Employees Insurance Health and Welfare Plan, reported that unknown individuals broke into their office and stole multiple items including paper records containing the protected health information (PHI) of 980 individuals. The PHI involved included names, dates of birth, and Social Security numbers. The CE notified HHS, affected individuals, and the media. In response to the breach the CE improved building security and stopped maintaining paper records in the building. OCR provided technical assistance regarding the HIPAA Rules.
