OCR description
The covered entity (CE), HCF of Bradford dba Bradford Manor, reported that its business associate (BA) experienced a hacking incident that compromised the protected health information (PHI) of 1,565 individuals. The PHI involved included names, addresses, Social Security numbers, lab results, diagnoses, and medications. The CE notified HHS, affected individuals, and the media. In response to the breach, the BA and CE provided complimentary credit monitoring and the BA implemented additional administrative and technical safeguards.
