OCR description
The business associate (BA) Bankers Cooperative Group, reported that it experienced a cybersecurity incident that affected the protected health information (PHI) of 14,403 individuals. The PHI involved included names, dates of birth, addresses, social security numbers, and claims information. The BA notified HHS and affected individuals. In response to the breach, the BA implemented additional technical and security safeguards.
