Texas Health and Human Services Commission

The covered entity (CE), Texas Health and Human Services Commission, reported that some of its workforce members inappropriately accessed, used, and/or disclosed the protected health information (PHI) of 94,261 individuals. The PHI involved included names, addresses, dates of birth, social security numbers, financial information, diagnoses, conditions, and other treatment information. The CE notified HHS, the affected individuals and the media. In response to the breach, the CE terminated access to its PHI, sanctioned the workforce members involved, and implemented additional safeguards to monitor access to PHI.
