OCR description
The covered entity (CE), Lucent Health Solutions, reported that an employee was the subject of an email phishing attack that affected the protected health information (PHI) of 37,000 individuals. The PHI involved included names, address, dates of birth, drivers’ license and Social Security numbers, claims information, diagnoses, and conditions. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented multi-factor authentication across all users, retrained workforce members, and implemented new technical and physical safeguards.
