OCR description
The business associates (BA), VectraRX Mail Pharmacy Services, LLC, reported that an unauthorized actor accessed its SQL server, which affected the protected health information (PHI) of 109,383 individuals. The PHI involved included clinical and demographic information. The BA provided breach notification to affected individuals, the media, HHS, and posted substitute breach notification online. In response to the breach, the BA provided complimentary credit monitoring services to the affected individuals, implemented additional administrative, technical, and security safeguards, and retrained workforce members to better protect PHI.
