OCR description
Catholic Charities West Michigan, the covered entity (CE)reported that it experienced a cyber-attack that compromised the protected health information (PHI) of 45,115 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license and social security numbers, health insurance information, diagnoses, and conditions. The CE notified HHS and the affected individuals about the breach. The CE also took corrective actions in response to the breach, implementing additional administrative, technical, and security safeguards to better protect its PHI.
