OCR description
The business associates (BA), U.S. Healthworks-SMMPP, reported that an unauthorized actor accessed its SQL server, which affected the protected health information (PHI) of 10,673 individuals. The PHI involved included clinical and demographic information. The BA provided breach notification to affected individuals, the media, HHS, and posted substitute breach notification online. In response to the breach, the BA implemented a variety of additional security measures and software upgrades.
