The covered entity (CE), Insurance Ace/Humana, Inc., reported that there was a mailing incident that affected the protected health information (PHI) of 8,553 individuals. The PHI involved included names, addresses, and diagnoses/conditions. The CE notified HHS, affected individuals, and the media.
