OCR description
The covered entity (CE), The League Treatment Center, reported that it was the subject of a cybersecurity attack that affected the protected health information (PHI) of 1,826 individuals. The PHI involved included names, Social Security numbers, addresses, drivers’ license numbers, dates of birth, diagnoses/conditions, medications, and other treatment information. The CE notified HHS and the affected individuals. In response to the breach, the CE implemented additional administrative, technical, and security safeguards, and retrained workforce members to better protect its PHI.
