Fillmore County Hospital

The covered entity (CE), Filmore County Hospital, reported that an employee was the target of an email phishing scheme that impacted the protected health information (PHI) of 3,535 individuals. The PHI involved names, addresses, dates of birth, social security numbers, claims and financial information, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE revised its relevant policies and procedures, increased its workforce security awareness training, and increased its threat protections to better protect its electronic PHI. OCR provided the CE with technical assistance.
