OCR description
The business associate (BA), HealthRev Partners, reported that an employee was the target of an email phishing scheme that impacted the protected health information (PHI) of approximately 1,446 individuals. The PHI involved names, dates of birth, and claims information. The BA notified HHS and the affected individuals. In its mitigation efforts, the BA enhanced its access controls and implemented multi-factor authentication and monitoring to better protect its electronic PHI.
