OCR description
The business associate (BA), Bassford Remele, reported that an employee was the target of an email phishing scheme that impacted the protected health information (PHI) of approximately 4,435 individuals. The PHI involved names, dates of birth, addresses, social security numbers, and diagnoses/conditions. The BA notified HHS, affected individuals, and the media. In its mitigation efforts, the BA provided security awareness training to its workforce and enhanced its access controls to better protect its PHI. OCR provided technical assistance to the BA.
