United Community Health Center

The covered entity (CE), United Community Health Center, reported that an employee was the target of an email phishing scheme that compromised the protected health information (PHI) of 750 individuals. The PHI involved included names, addresses, dates of birth, diagnoses, and medications. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE enhanced its system activity monitoring, increased its workforce security awareness training, and increased its threat protections to better protect its PHI. OCR provided the CE with technical assistance regarding the HIPAA Rules.
